Evaluation of cardiac sources of embolism
Abstract: Cerebrovascular disease is a leading cause of mortality and morbidity .20% of cerebral infarcts are due to cerebral embolism of cardiac origin. Conditions associated with cardiogenic emboli are either rhythm abnormalities such as atrial fibrillation (AF) responsible for 50% , or structural abnormalities .Whereas left ventricle thrombus ,left atrium thrombus ,vegetation , tumor and protruding atherosclerotic plaque are considered as direct sources of embolization , others such as valvular disease , wall motion abnormality , atrial septal defect (ASD) , patent foramen ovale (PFO) , atrial septal aneurysm (ASA) and spontaneous echocardiographic contrast (SEC) are considered as indirect cardiac sources of embolization . In this article evaluation of cardiac sources of embolism and the role of echocardiography are discussed. Though both transthoracic echocardiography (TTE) and transesophageal echocardiography (TEE) play an important role, TEE appears to be more sensitive in identifying potential abnormalities in patients with suspected cardiac sources of emboli.

Diagnosis and surgical management of a fetal cardiac tumor
Abstract: Female Infant was delivered by cesarean section at the University of Alberta Hospital secondary to concerns about fetal hydrops arising secondary to anterior Mediastinal tumor. There was apparently a significant increase in size of this tumor about 1 week before her delivery and the plans were therefore made to deliver the child at the University Hospital and if necessary, go directly to Surgery, to remove the tumor, which was done successfully two hours after delivery. Key words: fetal tumors; tumor surgery, endodermal sinus tumor, tumor cyst

Intracranial neoplasm presenting as Tachycardia-bradycardia Syndrome.
Abstract: 23 years old male presented with palpitation headache and syncopal attacks. Clinical examination was normal except for sinus bradycardia confirmed by ECG. Holter recording showed frequent episodes of tachycardia ending abruptly into bradycardia with heart rate varying from 175 bpm to 33 bpm. MRI of brain showed midline intracranial neuroepithelial cyst at the floor of fourth ventricle pressing on medulla oblongata. We conclude that this tachycardia-bradycardia syndrome is different from that of sinus nodal dysfunction and may be due to pressure on the vasomotor centre in the medulla oblongata. Pathophysiologic association of tachycardia-bradycardia syndrome and intracranial neoplasm is discussed.
